Physician Referrals

It is our policy for prospective patients to have a referral from their doctor in order to have pertinent medical records for review. Dr. Buzzanell gladly welcomes calls from referring physicians to discuss the chronic pain history in greater detail. You can also print the referral form (have link for PDF of a referral form) and have your doctor complete it and fax to my office for efficient consideration.
